Use the Cover tab to begin the student IEP. This tab records the
information that will display on the front page, including evaluation
dates, eligibility, program recommended,and IEP team members.
The Medical tab provides a list of items pertaining to services
provided by Health Assistants. The Medical Tab will display information
from the most recent finalized IEP, or if this is a new referral,
it will have imported the medical data from the Parent Input / Pertinent
Medical Information.
The PLAAP tab describes the students present levels of academic
achievement and functional performance. If the student is a Secondary
student and requires Transition Services, the Transition-based Assessment
section displays on the IEP. For Elementary students not requiring
Transition Services, skip to the Standardized Test section of this
tab.
The Special Cons. (Considerations) tab describes the student's
special needs such as assistive technology, behavioral needs, or if
they are English Language Learners.
Services will display the type, location and duration of special
education services provided to the student. If the service is a Medicaid
Billable service it will calculate the service hours.
The Testing tab documents whether the student will participate in
assessments with or without accommodations, or whether they qualify
for an alternate assessment.
The LRE (Least Restrictive Environment) tab describes to what extent
the student will have the opportunity to be educated with non-disabled
peers, what access they will have to the general education curriculum,
or what access to any other program that non-disabled peers would
be able to access.
The Parent St.(Statement) tab will print within the IEP. It is a
statement of the parent's awareness and acknowledgement of the student's
special education placement.
The Medicaid tab provides a list of items pertaining to services
provided by Health Assistants. assisting students that have health
related needs stated on the IEP.
The Attachments tab allows for the attachment of documents to the
MET Report (such as Compuscore printouts of testing, supporting documentation
from service providers, etc.) Documents included here will print at
the back of the MET Report as a single document, but will not have
continuous page numbers or headings.
