More Restrictive Placement Referral
Background:
Use the Background tab to document the reason for the referral along with the details of the student's attendance and placement histories. Family and home interactions can be documented here.
Tasks
Complete the Background Tab
If you haven't done so already,
Enter the
Document Date
(MMDDYY) or click
and select date. This should reflect the date the document was created.
Describe the
Reason for Referral.
Use

Attendance
Use the
Yes/No
to indicate if the student is attending school now.
Answer the attendance questions using a number format in the fields provided.
Placement
Specify
How long the student has been in a Special Education Program
and
How many periods spent in a special education class,
using number format.
Enter the
date of the most recent MET
(MMDDYY) or click
and select date.
List the types of
additional services provided on the student's IEP
.
Family/Home
Use the
Yes/No
to answer the questions regarding the student’s family and home.
Enter a numerical amount regarding the amount of non-custodial interactions.
Click
