Use the Cover tab to record the details of the evaluation, including,
the MET Date, the Team Members, the Type of Evaluation that
occurred, and additional eligibility information. This tab displays
the Three-year Reevaluation Due Date, also.
Use the Existing Data tab to document interventions attempted,
attendance and educational history, previous assessments, any impact
of educational disadvantage and or limited English proficiency and
any medical certification. (For Preschool students proceed to Existing
Data Tab - Preschool)
If the MET concludes that there is a need for additional data,
this is documented on the Existing Data tab. If such is the case,
the Additional Data tab displays. The Additional Data tab records
additional evaluation procedures, classroom observations, test behavior,
assessment results and other tests and findings.
The Eligibility tab summarizes the student's performance in general
education and documents the team's conclusions as to educational needs
and determination of eligibility.
Use the Additional Information tab to document any other relevant
and pertinent information regarding this student that has not been
covered elsewhere.
The Attachments tab allows for the attachment of documents to the
MET Report (such as Compuscore printouts
of testing, supporting documentation from service providers, etc.)
Documents included here will print at the back of the MET Report as
a single document, but will not have continuous page numbers or headings.
The MET Report Action Bar contains three Refresh buttons, Refresh
from Parent Input and Refresh from Referral located at the top of
the document.
Refresh Participants from Team is located in the MET Participants
section.
When the Refresh buttons are clicked, a confirmation dialog box
displays. Once confirmed, data from specified fields in the previous
document will import into the MET Report.
CAUTION: If any text
has been manually added to the fields prior to clicking the Refresh
button, the data will be over-written with the imported data.
Check Draft
to display the word "Draft" on all pages of the document
in the Print Preview option. Make sure to uncheck
prior to finalizing the MET Report.
Proceed through each tab and complete the
information.
