Consultation Request
Overview:
The Consultation Request contains two tabs:

for ConsultationThe
first part of the Request for Consultation tab is used to record the contact
information of the person requesting the consultation along with
the recommended time and location for the consultation to take place.
It provides a place to describe the reason the request was made.
The second part of this tab is used by the staff member receiving the request
to document the details of their observation.
