Incidents

Edit existing incident record

• Click .

• Select . This will change some fields from gray to white. White fields may be edited.

If student involved is a member of school in focus, two fields may be edited.

• Click Incident Role and select a different role.

• Check or uncheck Injuries Sustained.

• Click .

Add additional students to incident

• Click .

• Enter partial or complete data on any field on screen.

• Click . Search Results displays a list of records matching criteria entered.

• Click anywhere on appropriate record to highlight.

• Click  .                                  

OR

Click . This will move all records found in Search Results column to Selected Items column.

• Multiple searches may be made until all organizations have been found and added to Selected Items column.

• Check on line of student name to remove from Selected Items grid.  

• Click . This will close Chooser screen and student names selected will be listed in Students Involved grid.

• Click Incident Role and select student's role.

• Check Injuries Sustained, if appropriate.

Delete Students Involved

• Check on line of student name to remove.

• Click .

Add/Edit Other Involved Persons

• Edit directly in any white field.

OR

Click  on Other Involved Persons grid. A new line displays.

• Enter Last Name, First Name, Middle Name.

• Click Gender and select.

• Click Relation To School and select.

• Click Incident Role and select.

• Check Injuries Sustained, if appropriate.

• Click .

Delete Other Involved Persons

• Check on line of other involved person's  name to remove.

• Click .

NOTE: Go to Student Incident Discipline Detail to add/view additional details. Click on Students Involved grid in Synergy SIS.
