State Medicaid Reporting Code
Provider Type
Only the selected types are allowed to provide this service to a student.
Effective Date
Date that this service was added to Synergy SE.
Amount
End Date
(optional)
State Reporting Code
Unit
Choose from 15 MINUTES, 30 MINUTES, PER HOUR (.25, .50, .75, 1.0), PER VISIT, VISIT, and VISIT, PER PERSON
