Health Screen

Use the Tuberculosis tab to record detailed results of up to two TB skin tests, including: type of test, date it was given, date it was read, and results of test.

Use the Vision tab to record all vision screenings for a student, including: date and grade level of screening, results of right eye, left eye, with and without glasses, color deficiency, ocular alignment, and any referral information.

Use the Hearing tab, to record all hearing screenings for a student, including: date and grade level of screening, reason for screening, detailed results, referral information and an area for written comments.

Use the Scoliosis tab to record scoliosis examinations for a student, including: date, grade level, and result of physical examination. There is a place for referral date and dates of x-rays. Additional comments may be added.

Use the General Health tab, to record all general health information for a student, including: date and grade level information was taken. It includes: height, weight, body mass index (BMI), heart rate, and blood pressure. If student is referred to a medical professional for follow-up, date of referral is shown. This tab displays any health or drug tests performed for student group participation, as well.

Use the Dental tab, to record details of a student’s dental screening, including: date, grade level, and result of screening, visible cavities or fillings, treatment recommended, reason for waiver, and follow-up date, if needed.

HLT211 – Health Screening Profile

The HLT211 report will show all the data that exists in the Health Screening screen, based on the parameters selected on the report interface.
