• Enter Initial Care Given/By Whom.

• Check Medical Care Recommended, if appropriate.

• Enter a location in Taken Where After Accident (Specify Home, Hospital, etc.).

• Enter name in Taken By.

• Enter Time Taken. It is only necessary to enter numerals. Synergy SIS formats field once entered, i.e. 900 would be formatted as 9:00 AM.

• Enter Reason Injured Person was on the Premises (lunch, P.E., etc.).

• Enter Witnesses names.

• Enter Follow Up plan.

• Enter Preventative Measures Taken, if appropriate.
