Enter Accident Date (MM/DD/YY) or click and select date.
Enter Accident Time. It is only necessary to enter numerals. Synergy SIS formats field once entered, i.e. 900 would be formatted as 9:00 AM.
Enter Initial Care Given/By Whom.
Enter Accident Location.
Enter End Date, if there is one, or click and select date.
Click next to Supervising Staff Member. Find Staff screen displays.
Enter all or part of Last Name, First Name and/or Middle Name.
Click . Staff names matching criteria entered are listed in Search Results grid.
Click Staff Name to highlight.
Click . Find Staff screen closes and Supervising Staff Member name displays.
Check Medical Care Recommended, if appropriate.
Check Student covered by School Accident Insurance, if known.
Enter a location in Taken Where After Accident (Specify Home, Hospital, etc.).
Enter name in Picked Up By.
