Accident Detail tab

Once a Nurse's Log Entry has been saved, additional details may be added or edited on the Accident Detail tab.  Use the Accident Detail tab to record additional Nurse's Log entry information if the incident was an accident. It records date, time and location of accident and, if appropriate, an end date.  It also shows who initially assisted student and staff member who supervised medical treatment.  Additional details that may be recorded include status of student’s accident insurance, if additional medical care was recommended, when and where student was taken afterwards and who picked up them up. The Accident detail tab records why student was at location, any witnesses to incident, follow-up care needed, and any preventative measures taken to avoid future incidents.

A letter and form detailing an accident called, Student Accident/Incident Report, can be printed on this tab. The report displays a cumulative list of a student’s accident/incident records including: accident information, accident location, parent contact, and a detailed injury description. Parent may be given this report when a student has an accident/incident on school grounds. Additionally, the principal and nurse can sign this form on provided signature lines. The Student Accident/Incident Report displays in a separate PDF window.  This report can be generated from the report interface located, Synergy SIS>Health>Reports>Individual, HLT203, also.
