Complete Request
Enter student
Last Name, First Name,
and
Middle Name.
OR

Perm ID.

Enter or edit

and
Email
of person requesting record, if appropriate.
Click
State
and select location of district and school where request will be sent.
Click
District
and select destination district.
Click
School
and select destination school.
